Years later I found out Tommy was suffering from Schizophrenia with paranoid delusions and the D&D world was all around him in real life. It still is actually as he is quite resistant to inti-psychotic drugs/therapy. We can at least choose to be in this world and treat it as a game. Tommy can't, he suffers his inner demons without the choice to bail out. No game over for him.
And very sad story, too.
A nice DM could actually make his life easier, couldn't he?
("Tim the paladin escapes the trap easily and unharmed!")
But what hell he would experience if the DM hates him...
